Sneha Sandhya Age Care Foundation is a Registered Charitable Trust established in 2012 with a vision to provide Geriatric care to underprivileged elders and Palliative Care to persons suffering from life limiting illnesses. Our mission is to deliver high quality Geriatric, Palliative and Dementia care.
We organize outreach services to the elderly in the community, old age homes and the terminally sick people especially those suffering from cancer, right at their door step.
We sensitize, inspire and empower individuals and organizations to increase access and optimize the practice of palliative care and dementia care.
We provide institutional support to persons suffering from life threatening illnesses like cancer. Our dedicated team of doctors and nurses, trained in palliative care, serve the in-patients to lessen their pain and suffering thereby improving their quality of life.
We build capacity through education and training; promote similar services across the state through awareness creation, advocacy and partnerships with government, private health sector and other non governmental organizations working in the areas of geriatric/palliative/dementia care.
We have a community participatory approach to our work, involving a large group of committed volunteers, sensitized and trained in palliative/geriatric/dementia care practices.
CORE VALUES
Patient First: We believe that the patient comes first in our order of priorities followed immediately by his/her family.
Compassion: We will work with compassion in all our dealings with patients, their families, our staff and the community we serve.
Community: We recognize that community is the foundational fabric of our society. We hope to build strong, lasting and loving partnerships with our colleagues, volunteers and community entities to work with us as one team, meeting all our community’s Supportive Care needs.
Empowerment: We acknowledge that power differentials in healthcare can make it difficult for individuals to ask for their needs to be met. We will give individuals information and resources that help them advocate for themselves and their families. We will facilitate / provide Information so that patients and families are enabled to make informed decisions.
Equity: We recognize that there are significant disparities in our community and within the provision of healthcare services. We will pay careful attention to them when we design and deliver our services.
Integrity: We commit to conduct our work honestly and strive to be dependable, forthcoming and open. As we work with communities, both within and outside the organization, ours will be an attitude of humility, acknowledging the limits of our own expertise. We will strive to build integrity into every aspect of our work.
Dignity: We recognize that every individual has a right to be treated with respect and dignity even when their goals, values and priorities may be different from our own. We will help to educate and build conditions that uphold the dignity of all patients.
Quality and Excellence: We will work to inculcate excellence in everything we do and build, and continuously improve quality in the services we provide, while ensuring that each and every patient receives the same quality of care, irrespective of their social or economic status.
Sustainability: All our efforts will keep sustainability and environmental protection in mind at all times
